P2P-Client ACH MONTHLY PAYMENT AUTHORIZATION AGREEMENT
ACH PAYMENTS POLICY This Automatic Recurring Agreement (the “Agreement”) is a contract between you (the “client”) and Whitney Larson (dba Pain2Power Coaching, P2P) to set up your automatic recurring payment. Please verify that all information provided is complete and accurate before submitting this agreement.
FREQUENCY OF PAYMENTS An automatic recurring payment will be made monthly after your initial payment. Payments will be scheduled based on the date of your initial payment unless specified by the client. An email will be sent to the client’s provided email address 3 days prior to payment due date. The client will receive a payment confirmation email following all automatic recurring payments.
AUTOMATIC RECURRING PAYMENT FAILURE If recurring payment is declined or unable to be processed for any reason, the client will be contacted by phone, text, or email. In the event any payment is not made and the client does not make the required payment by the end of the agreed billing cycle, the client is responsible for the contracted full total balance and remainder training agreement terms.
CHANGING AUTOMATIC RECURRING PAYMENTS If the client needs to change payment methods or billing information they will provide those changes through CLIENT BILLING PORTAL.
IN-PERSON TRAINING SESSION CANCELLATION POLICY P2P operates on a scheduled hourly appointment basis for private training sessions. Therefore, when canceling an appointment, it is required for the client to provide MORE than 24-hour notice, for which no additional fee shall be incurred.
However, if the client cancels an in-person session LESS than 24 hours to the scheduled time, the client shall be charged $20 per late cancellation fee. Coach agrees that the client is entitled to a one-time cancellation fee waiver per month, whereby they can cancel a session in less than 24-hours, at no additional charge.
If the client fails to give any prior notice to the scheduled in-person training appointment session, the client shall be charged $40 per no-show fee.
ONLINE SESSION CANCELLATION POLICY P2P online programs are scheduled via phone call check-ins. Therefore, weekly appointments are crucial to the success of the client’s progress. When canceling an appointment check-in call, it is required for the client to provide MORE than 24-hour notice, for which no additional fee shall be incurred.
However, if the client cancels a weekly check-in appointment more than 2 (two) consecutive weeks, the client shall forfeit any investments to their program. Coach agrees that the client is entitled to a one-time cancellation, a reschedule weekly check-in call at no additional charge.
I, hereby authorize Whitney Larson (dba Pain2Power Coaching) to initiate a recurring electronic debit from my provided payment method. I agree to provide Pain2Power Coaching with accurate payment information during our agreed terms, and will provide a 30-day cancellation notice prior to the cycled billing date for ample time to terminate ACH billing charges.
